The fee for an initial eating disorder evaluation is $350.00 and is between 1.5 and 2 hours in length. . The initial consultation fee is typically $250.00 for other clinical issues and lasts about an hour. Sessions are typically $225 for a 45 to 50 minute session. However fees do vary depending on services provided.
I am in-network only for United Health Care and Optum commercial plans. Please call UHC or Optum to verify that I am in network for your plan. When calling the insurance company they might ask for my NPI # which is 1982823530. If I am in network I will submit the claim for you. You are responsible to find out your benefits, deductible and copay information. Please note that not all services are fully covered.
If I am out of network for your insurance plan then payment is requested at the time that services are provided. I will provide you with a receipt with all of needed information should you choose to submit it to your insurance for reimbursement. It is your responsibility to find out if you have out of network benefits for mental health services.

If you do not show up for your scheduled therapy appointment, and you have not notified the office at least 24 hours in advance, you will be required to pay the full cost of the session. Exceptions are only for unexpected emergencies. Notice for Sunday cancellations needs to be given no later than Thursday afternoon.
